The Caregivers Notebook



Recently I was talking to a caregiver and they shared they just have trouble trying to get everything done and stay organized”. I thought about this for a few weeks and the idea of a caregiver notebook was born.


The Caregivers Notebook was created to help caregivers keep track of appointments, medications, weights, vitals, behaviors, and the disease progress.



How can the Caregivers Notebook help you?



The Caregivers Notebook can be taken to doctor’s appointments with you and will help your doctor see a better picture of your loved one’s progress. The doctor will be able to see a years worth of your documentation. This will help him better manage the plan of care for your loved one.

Often visits to a doctor's office are brief and vague. A spouse may tell the doctor "he acts out all the time". This does not give the doctor a very good picture of what the home situation is really like. Your idea of all the time and my version of all the time could be two completely different amounts of time. One could be several times a day and the other could be a couple of times a month. When you have the changes written down on the tracker form, they become very clear to your doctor. This can help him determine if a medication needs to be added or a change is needed to medicine already being taken.

Another tracking form in the Caregivers Notebook tracks the progression of the disease, which helps everyone involved provide the appropriate care. It also helps to recognize when it is time to add more help in the home, whether that is medical equipment or extra helping hands.

   This notebook is also designed for family or sitters who can sit with your loved one while you take time for yourself. This book should be their resource to help them provide the same care you would. This will allow you to step out of your caregiver role for a while, but have peace of mind because your fill-in has the necessary information needed to provide the care.
